Is it true that a psychiatric residency can only last 3 years if you opt to take a fellowship fourth year?

Like say for instance I wanted to take fellowships in consultation liason psychiatry(now called psychosomatic medicine) and forensic psychiatry.Could I replace my PGY4 with one of these actual fellowships If I emphasize the fact that I don't want to be a general psychiatrist,but one who specializes?
